Western New York Integrated Care Collaborative (WNYICC) Program
- WNYICC's Falls Prevention Program is designed to empower individuals with a history or fear of falling to address social factors occurring in their home in order to better manage their care and improve their quality of life, through the assistance of a Health Coach over 6 months.
- Participants receive one-on-one coaching (including falls risk assessment and care planning) PLUS enrollment into an evidence-based Falls Prevention group workshop such as Enhance Fitness, Chronic Pain Self-Management, Matter of Balance, Walk with Ease, and Tai Chi.
