MediSource  

MediSource is a New York State sponsored Medicaid Managed Care program for individuals with no health insurance who qualify for Medicaid. Independent Health’s MediSource plan allows you to choose your Primary Care Provider (PCP) and women can also choose an OB/GYN. Your PCP will provide your health care and refer you to other doctors if you need them.

Child Health Plus  

Child Health Plus might be right for you if your child has no health insurance coverage. Eligibility is determined by NY State of Health, The Official Health Plan Marketplace. Your income and the size of your family determine your monthly premium for Child Health Plus.

The Essential Plan  

The Essential Plan is a program for eligible New Yorkers with lower incomes that do not qualify for Medicaid or Child Health Plus. The NY State of Health’s Essential Plan includes comprehensive benefits and has no deductibles, so the plan starts paying for your health care right away.

MediSource Connect  

MediSource Connect is a New York State approved Health and Recovery Plan, otherwise known as a HARP. A HARP is a Medicaid managed care insurance plan for adults age 21 or older that manages physical health, mental health, and addiction services in an integrated way.
