Thank you for using Independent Health's mobile application. To view your personal member information using our mobile application, you must first register on our website. Through the registration process, you will create a Username and Password to be used to access Independent Health's secure features of our mobile application. When you register, your information is stored in a secure environment and is protected behind a firewall. The information that is transmitted back and forth is encrypted.
Information we collect
While using our mobile application, we may collect the following information:
Device information
We may collect device-specific information such as your hardware model, operating system version, and mobile network information.
- Log information
When you use our mobile application, we may automatically collect and store certain information in our system. This may include:- Your device’s Internet Protocol (IP) address. This is the number that is automatically assigned to your device whenever you connect to the internet.
- The network provider which you access the Internet. For example: Verizon or Sprint.
- The date and time you used our mobile application or accessed the mobile applications services and how long you spent using the mobile application.
- The pages you looked at or services you accessed.
- The name and version of your device’s operating system and, if applicable, web browser.
Local Storage
While using our mobile application, you may ask us to “remember you” during the login process. By selecting this feature, the mobile application will store your username on your mobile device in an encrypted file.
Remember This Device (Multi‑Factor Authentication)
After successfully authenticating with a username, password, and MFA, you may optionally choose “Remember this device for 24 hours.” By selecting this feature, your username is securely associated with the specific device or browser, allowing MFA to be bypassed for subsequent logins during that period. To support this functionality, we securely store your username and a unique identifier for your device or browser.
Communication Preferences
These are your preferences for how Independent Health communicates to you regarding specific topics or programs. You can consent to receive these communications through mail, email, SMS messaging, or automated phone calls. Your consent is strictly limited to communications directly from Independent Health on the channel you choose. We will not sell or share your email address, telephone number, name, mailing address or any other personal and non-public information with any telemarketer or marketing agency.
Data Retention
Independent Health retains member data collected through the mobile application for as long as the member maintains an active portal account, or as otherwise required to comply with applicable federal and state laws, regulatory requirements, contractual obligations, and internal recordretention policies. Certain information may be retained for a longer period when required for legal, regulatory, auditing, security, or operational purposes, even after an account is closed.
- Data Deletion and Account Closure
Members may request deletion of their Independent Health portal account and associated mobile application access at any time. Account deletion can be requested in one of the following ways:- By contacting Independent Health Member Services by phone.
- By submitting a request through the Login & Security section of the member profile within the mobile application. Once a deletion request is processed, access to the mobile application will be removed. Certain data may be retained as required by law, regulation, or internal compliance obligations.
How we use information we collect
We use the information we collect from our mobile application to provide, maintain, protect and improve the application, to develop new ones, and to protect our members and our users. We do not share personal information with companies, organizations and individuals outside of Independent Health.
Changes
Our mobile application security statement may change from time to time. We will post any privacy policy changes on this page and within our mobile application and, if the changes are significant, we will provide a more prominent notice.
